Cystatin C

Kidney disease is one of the most common diseases in the world and is a major public health problem in the world including Thailand. Referring to Ministry of Public Health’s data, in 2015, approximately 8 million Thai people or 17.6% of Thai population suffered from Chronic Kidney Disease. Kidney disease can be categorized into 2 groups: acute kidney disease (few cases) and chronic kidney disease (most cases). The major risk factors for chronic kidney disease are diabetes, high blood pressure, obesity, high blood lipid levels. The diseases can be found more in people age over 60 years.)

Cystatin C is a protein in the blood that is filtered by kidneys and acts as an indicator of kidney function. It is found in both tissues and fluids in the body which prevents connective tissue from being destroyed by enzymes that work within the cells and play a role in preventing bacterial and viral infections. In normal people, Cystatin C is released into the bloodstream in constant volume and is filtered through glomerulus, which refrain Cystatin C  to reverse into the bloodstream. If renal and glomerular filtration does not function well, Cystatin C levels in the blood will beincreased.

Comparing Cystatin C with Creatinine in early stage of kidney disease:
Cystatin C is a better marker than creatinine in early stage of kidney disease. Cystatin C can show early renal abnormalities better than creatinine because of its better precision and clinical sensitivity to renal function and better correlation with eGFR (estimated Glomerular Filtration Rate). Besides creatinine is affected by muscle mass and food intake. Creatinine is also insufficient in early stage of kidney disease surveillance, as it is detected when kidney function is damaged by 50 %.
